Home Health Services
The home care team, composed of RNs, LPNs, Physical Therapists, Physical Therapy Assistants, Occupational Therapists, Speech Therapists, Medical Social Workers and Home Health Aides, works with the patient´s physician to provide the following services:

Adverse drug events cause over 700,000 emergency department visits each year, and nearly 120,000 patients each year need to be hospitalized for further treatment after emergency visits for adverse drug events. Older adults are twice as likely as others to come to the ER for adverse drug events and almost seven times more likely to be hospitalized after such an emergency visit (Centers for Disease Control and Prevention).

Guardian Monitoring service gives you the confidence and freedom to live on your own, because there´s always someone standing by to help whenever you need it, 24-hours-a-day, 7-days-a-week, all at the press of a button. Our technologies to promote independence include:
• Water resistent pendant with a medical alert button
• Dials into Guardian Call Center at the push of the button
• Guardian operator places call on caller's behalf to appropirate person (facility on-call, family member, neighbor and/or 911)
Automated Medication Dispensing Units
Technologies are available to improve medication adherence so that you or your loved one take(s) the right medications at the right time, everytime.
The larger system announces a reminder, "Time for your medicine, press button to dispense." Once you push the large blue button, the right dose of medication is delivered. If you miss any doses, your caregiver is notified. The unit is also kept safe and secure - it is locked to prevent from tampering. This unit requires to be plugged into a telephone line and a power outlet, and it contains back-up battery in case of power outages to make sure the service is uninterrupted. It also notifies you when the batteries require replacing, and our technicians can be dispatched to your home to replace them for you. In addition, technicians come to your home for the initial set-up of the equipment to make sure that it is in working order.
The smaller, portable device - MedMinder - sounds an alert, and the appropriate compartment illuminates when it is time to take your medication, making it ideal for low hearing and low vision subscribers. If the medication is not taken when prompted, or the wrong compartment is opened, a caregiver is notified. The reminders are easily programmable. It holds upto four doses a day and seven days of medications.
